I-94 samples
Form I-94 is the record of your entry into the United States.
It shows the date of entry, your status, and the expiration date of that status.
How to obtain
Form I-94 is issued by a Customs and Border Protection (CBP) officer
at a U.S. Port of Entry.
If you entered through the Mexican border and were not given
a paper I-94 or I-94A,
most likely you will no longer receive one.
However, you can try to find your form
on the official website:

GET MOST RECENT I-94 -> I ACKNOWLEDGE AND AGREE -> enter your information ->
In the “Document Number” field, enter your A-number [instead of your passport number]
What to do if your I-94 shows “Not Found” online:
1. Try different spellings of your first or last name. An error may have been made
2. Did you enter your A-number instead of your passport number? Try entering your passport number
3. If you entered your first name and middle name and it was not found,
try using one name or the other.
Also try entering your first and middle name in the first name field.
4. If you were born between the 1st and the 12th of the month,
try switching the day and month of birth,
as an error may have been made.
If you still cannot find your I-94 form,
you may contact the Traveler Communications Center (TCC)
of U.S. Customs and Border Protection (CBP).
They should respond within 72 hours:

How to correct an error
If CBP issued your Form I-94, I-94W, or I-95 with incorrect information,
you must appear in person
at the nearest CBP Port of Entry
or the nearest CBP Deferred Inspection Office
to have the information corrected.

USCIS will not correct an error on an I-94 issued by CBP!
How to extend
If you entered through Mexico,
you will most likely be denied an extension of I-94.
In practice, almost no one succeeds.
However, there are a few known successful cases,
and you may try to do this
at the nearest
CBP office.
You must appear in person and request an extension.
It is possible that extending I-94 will become standard practice in the future,
but for now you should not rely on it.
The second option is to try filing
Form I-539
with USCIS.
However, first, a $370 filing fee is required,
which is non-refundable even in case of denial,
and second, it is primarily intended
for other categories and statuses,
so the chances are also low.
If you decide to try,
you should file Form I-539
at least 45 days before your I-94 expires.
White Card. I-94 after a successful court case
This is another type of I-94.
To obtain an I-94 after being granted asylum,
you must schedule an appointment
at the nearest USCIS office
by calling 1-800-375-5283.
Tell the automated system that you need InfoPass.
Be prepared to speak English
or have an interpreter with you,
as USCIS does not provide interpreters by phone.
Unfortunately, if you are located in the United States,
you can no longer schedule an appointment online
using InfoPass for domestic offices.
